Trouble getting new US Passport

Hey, I applied for my passport book in 8/18 (submitted all documents required) was told 4-6 weeks and this past week got a letter from their Chicago office saying that was insufficient information for applying for a US passport. Has anyone else ran into this problem? Also I have a class A CDL with Hazmat which REQUIRES Federal background screening with finger prints!
